We often call it appendicitis, but it is a misnomer, and the appendix refers to the part of the large intestine where the appendix comes out, and the correct expression should be called appendicitis.
Although the cause of appendicitis is not clearly known, it is known that most appendicitis begins when the opening of the appendix is closed. In the case of teenagers, it mainly occurs because the entrance is blocked due to the proliferation of submucosal lymphatic vesicles.
Abdominal pain is usually accompanied by anorexia, nausea, and vomiting (80%) with localized abdominal tenderness and fever. Abdominal pain is initially vague in the upper abdomen, then gradually localizes to the right lower abdomen. The symptom most often complained of by patients who came to the hospital with appendicitis is, “My stomach hurts and I can’t digest it, so even though I ate digestive medicine, the symptoms continue, and when I walk, my lower right stomach feels stiff and hurts when I press it.” However, not all patients visit the hospital with these typical symptoms.
Depending on the location of the appendix, there may be pain in the right flank, and if it is located in the pelvis, post-experimental syndrome (a symptom of wanting to defecate after defecation) and vague discomfort in the lower abdomen may appear. Therefore, even if it is not a typical symptom, if you have abdominal pain, you should be carefully examined and tested.
Characteristic clinical symptoms and physical examination are the most important in the diagnosis of appendicitis. In addition, blood tests and abdominal ultrasound or abdominal CT may be additionally helpful.
There are diseases that show symptoms similar to appendicitis, so it is necessary to differentiate them. In children, it is necessary to differentiate between acute mesenteric lymphadenitis and intestinal intussusception. In addition, diverticulitis, ulcer perforation, and acute cholecystitis can show similar symptoms. In the case of the elderly, since symptoms and physical findings may be non-specific, caution is required.
The treatment of appendicitis is surgical removal of the appendix.
Since the aftereffects of neglect are much more serious than the complications of surgery, active surgical treatment is necessary when appendicitis is suspected. Recently, most appendicitis operations are performed laparoscopically.
