Precautions for filling out an infant check-up
Please make sure the date of your appointment and the number of months of developmental screening are correct.
Please make corrections and check again before the appointment date for the examination.
Even if you fail to fill out 3 or more domain questions, the developmental screening test proceeds, but the test is not accurate (informed by the corresponding pop-up)
The ‘additional questions’ are mandatory and the developmental screening test cannot be performed if at least one is not filled out.
